Identity + NPI + state + per-measure rates and footnotes. **NPI IS PRESENT** (100% coverage) — the only Care Compare dataset that publishes NPI directly on the agency-level table.

What are the ASC quality measures?
CMS measures ambulatory surgical centers on measures ASC-1 through ASC-12, covering: patient burns (ASC-1), patient falls (ASC-2), wrong site/side/patient/procedure (ASC-3), prophylactic antibiotic timing and selection (ASC-4 and ASC-5), appropriate antibiotic discontinuation (ASC-9), hospital transfer rate (ASC-11), and outpatient surgery experience (ASC-12, CAHPS). Each measure is expressed as a percentage with a national benchmark.
Where can I download the CMS ASC data?
